AIA Case Follow-up Health Management Event
This case follow-up event is an exclusive planning program of AIA Life. The program will be implemented for six months. The case follow-up health management program will work with the Huston-Apollo Model telemedicine team of the National Taiwan University Hospital Yunlin Branch to jointly guide and implement health assessment and health promotion plans for participating members.

Case follow-up program rewards
Complete the designated tracking tasks within half a year (see the minimum measurement requirements for designated tracking tasks below ) and rent medical equipment for free without give back.
If any designated tracking indicator of an individual improves by 20% from the normal value, a NT$3,000 gift certificate will be awarded on a selective basis; the so-called normal value generally refers to any normal value range recommended in the physical examination report, the same below. Please refer to the case follow-up event description 5 for the reward criteria.
If any of the individual's designated tracking indicators returns to normal value range, an additional NT$8,000 gift certificate will be awarded

Specify the tracking task:
Track health indicators as recommended by doctor, which may include measurements of blood pressure, blood sugar, weight, physical activity, calorie intake, etc. For the correct use of medical devices such as blood pressure monitors and blood glucose meters, please refer to the instruction manual or contact the medical device manufacturer, health care personnel or contact us to assist you in using the relevant equipment correctly.
Blood pressure: It is recommended to measure blood pressure once in the morning and evening every day, or at least once a month at a fixed period (such as the beginning, middle or end of the month), for 7 consecutive days, measuring once in the morning and once in the evening .
Blood glucose: It is recommended to measure fasting blood sugar regularly every day, or at least once a month at a fixed period (such as the beginning, middle or end of the month) for 7 consecutive days .
Body Mass Index (BMI): It is recommended to measure your body weight every day on an empty stomach, at least twice a week . BMI = weight (kg) / height (m) squared
Physical activity: It is not a necessary tracking item. If necessary, you can choose to use a health wearable device and use the health app to track the records.
Calorie intake: It is not a necessary tracking item. If necessary, you can use the health app to track the records.
This event is open to 10 people.
Purpose:
Assist eligible cases to track participants’ health test value improvements and conduct health management follow-up. For the eligibility criteria of participants, please refer to the case tracking activity descriptions 2 and 3 below.

AIA Case Follow-up Health Management Program Description
The AIA case tracking health management program is mainly to assist AIA Life group insurance policyholders in providing health management services in order to achieve the goal of health promotion.
According to the theme of this year's event, cardiovascular disease prevention generally refers to those who have been diagnosed by medical institutions or consciously meet the National Health Administration's chronic disease prevention and control guidelines for diabetes, hypertension, hyperlipidemia, kidney disease, smoking, alcoholism, obesity, lack of exercise, and those who have or have family members who have had a stroke or ischemic heart disease.
The maximum number of participants for this event is 10 people. After registration, people who have undergone qualification review by AIA Life and are assessed by doctors appointed by the organizer or cooperative medical institutions to have non-emergency conditions and meet the qualifications in Article 2 for sub-health or chronic diseases are eligible to participate. If the number of registrants for the event exceeds the upper limit, doctors appointed by the organizer or cooperative medical institutions will select candidates for the event.
During the case tracking activity, the organizer will provide free physical health examinations required for activity planning, rental of physiological measurement medical equipment, application software required for case tracking, and opinions and suggestions on health management tracking from medical staff. A self-provided health examination report within one year can be used as a substitute for the pre-activity physical examination report, and a post-activity report can be replaced by a health examination report from a medical institution within two months .Medical equipment may include a blood pressure monitor, a weight scale, a blood glucose meter and 50 consumables required for the blood glucose meter, as directed by the doctor.
Reward activities: In order to improve the health management effect of participants, participants who complete the co-organizer's questionnaire at the end of the event will receive rewards if they meet the qualifications. The planned reward activities are as follows:
(1) Complete case tracking: If the case tracking task is completed above the minimum standard as instructed, the rental equipment will be provided free of charge during the case tracking period and does not need to be returned.
(2) Health Management Effectiveness Rewards: Physical examination results before and after the designated health management tracking items are used. A 20% improvement in the health range will provide a NT$3,000 gift certificate reward. If there are no abnormalities in the physical examination results at the end of the activity, an additional NT$8,000 gift certificate will be awarded. All performance reward data is based on the physical examination report arranged by the organizer or the physical examination report of a compliant medical institution. If there are more than two designated tracking indicators, rewards will be given based on better one improved result, with the maximum reward amount per person being NTD 11,000.
If the free rental medical equipment does not meet the free return reward of item (1) of the follow-up description in point 5, no fee is required when returning it as long as the detection function is normal and the accessories are complete. Otherwise, it will be regarded as purchasing a new device with a tax-paid price of NT$6,500. Blood glucose consumables do not need to be tested. If there are any leftovers, they can be returned directly.
